Medical Requirements - Driver License

General Information

Drivers are reviewed in response to self-reported conditions and reports received from law enforcement, medical professionals, MVD offices and concerned citizens. These reports are reviewed to determine if the driver requires a re-examination of driving skills or written testing, or medical or psychological evaluation.

It is the responsibility of the driver to report to MVD any medical condition that may affect the ability to safely operate a motor vehicle. This information must be reported (in writing or by phone) as soon as the medical condition allows.


Law Enforcement Reporting

We receive information from law enforcement regarding possible unsafe drivers. If a driver is stopped by law enforcement but is not issued a ticket, it does not mean the officer cannot request the driver to be reviewed by the Medical Review Program. A law enforcement officer may report a driver by completing the Re-Examination Request, which may also be used by MVD personnel.


Physician Reporting

Even with the physician-patient, nurse-patient or psychologist-client confidentiality relationship, a physician, registered nurse practitioner or psychologist may voluntarily report a patient to MVD who has a medical or psychological condition that in his or her opinion could significantly impair the person’s ability to safely operate a motor vehicle.

The information must be in writing and must include the name, address and date of birth of the patient. The medical professional submitting the report in good faith is immune from civil or criminal liability for making the report.


Concerned Citizen Reporting

A person with direct knowledge of a driver’s specific conduct or specific events indicating the driver may have a disqualifying medical condition may report this to us by completing a Driver Condition/Behavior Report.


Reporting an Unsafe Driver

If you have direct knowledge of a person’s medical status that would affect his or her ability to safely operate a motor vehicle you may complete a Driver Condition/Behavior Report.

Information received by us is confidential and cannot be released.

Once we receive the form, our only contact will be with the driver. We will not notify the reporting person of any outcome.


Medical, Vision, Substance Abuse Requirements

If you have received a request for a medical review, it means we have received information regarding your ability to safely operate a motor vehicle. We receive information from law enforcement, medical professionals, insurance companies and concerned citizens. The source of the information we receive is confidential, and cannot be released.

You may also receive a request if you mark yes to the medical question on a driver license application, or by calling and self reporting a medical condition, in this case the forms will indicate “self reported”.

The reports must be completed by the type of medical professional listed on the form. We cannot make referrals.

If the requested information is not received within 45 days of the notice date, your driving privilege will be suspended and/or revoked until the information is received and reviewed.

Incomplete forms will delay the process. We will send a request for additional information if the form is incomplete or if the form does not address the reported condition.

Once we have received all the required information, we will mail you one of the following:

  • Driver License Medical Eligibility Notice – If you are eligible. You may be required to add restrictions, be informed of a future follow up and/or be notified of requirement of a vision, written and/or road test. Only the boxes marked on the notice apply to you.
  • Summary Suspension Order and Revocation Order –If you are not eligible.
  • Driver License Medical Ineligibility Notice and Revocation Order – If you are already suspended or revoked and still not eligible, after turning in new information.

Vision Test, Written Test, Road Test

Request for standard testing:

If you are required to complete a vision, written or road test, you may visit any MVD Office during normal business hours to complete the required testing. You do not have to schedule an appointment.

Failure to comply within 30 days from the date of the notice will result in the suspension and/or revocation of your driving privilege.

Request for Special Performance Evaluation (SPE):

If you are required to complete a Special Performance Evaluation, you must have a scheduled appointment due to the extended time this evaluation takes. An SPE examiner will contact you for scheduling.

You may also complete the SPE with one of the following Certified Driver Rehabilitation Specialists (CDRS).

The SPE must be completed within 30 days from the date of notice whether testing with MVD or a CDRS. Failure to comply will result in the suspension and/or revocation of your driving privilege.


Suspension and Revocation

Reasons for receiving suspension and revocation notices:

  • Failed to comply with medical requirements
  • Failed to meet medical and/or vision standards
  • Failed to comply with a request for a vision, written or road test
  • Ruled incapacitated by a court

If your driving privilege is suspended and/or revoked for failure to submit a vision examination, medical examination, substance abuse evaluation, special performance evaluation certificate of completion (SPE COC), or a driving evaluation, you may submit the information by fax to 623-925-9323 or by e-mail (see Contact Medical Review). For any other suspension and/or revocation from the Medical Review Program, please call 602-771-2460 for further information.

You have the right to request a hearing on these actions, however, such request will not stop the immediate suspension. Your request must be received within 15 days from the date of the notice. The request must be made in writing and may be made by any one of these methods:

The request must include your full name, date of birth, license number, mailing address, home and business phones, and Case Number. Upon receipt of your written request, a hearing may be scheduled and you will be notified as to the date, time and location.

It is against the law to operate any motor vehicle on the public roads of this state while your driving privilege is suspended. If such operation results in your arrest and conviction, the suspension will be extended for up to one additional year.


Window Tinting Exemption

Arizona law states the amount and manner of window tinting a motor vehicle may have to be lawfully operated in this state. It is a violation of law to operate a motor vehicle that does not meet these window tinting requirements. Specific questions should be directed to your local law enforcement agency.

If you are required for medical reasons to be shielded from the direct rays of the sun, you may receive an exemption from the window tinting requirements. The exemption is granted for any vehicles that you own, as well as other vehicles in which you are a regular passenger.

To apply for the exemption, present a letter, signed by a physician, stating the medical necessity for window tinting and a copy of the registration to all vehicles to be tinted.

The letter and registrations must be submitted to the Medical Review Program for evaluation.
