Insurance Requirement Exemption

By submitting this request, you are certifying that:

  1. You are the owner/lessee or controlling operator of this vehicle.
  2. That the vehicle will not be operated on any public roadway in Arizona at any time after submitting this request.
  3. The vehicle will not be operated in Arizona until proof of insurance is on file with the Motor Vehicle Division. This may be completed at any MVD or authorized Third Party office.
  4. You understand that you must still pay any registration fees and taxes required.
