Executive Hearing Web Portal

Use the web portal to do any of the following:

  • Request a hearing
  • Request a summary review
  • Request to cancel a hearing
  • Request a rehearing
  • File a Notice of Appearance (attorneys only)
  • Request a continuance
  • Submit a stipulation
  • File a motion
  • Make a general inquiry

To use the web portal you will need all of the following information:

  • Full name and date of birth
  • Valid email address
  • Mailing address

You should also have the following information when available:

  • Phone number
  • Driver license

NOTE:  You will receive an email confirmation of your submission within 24 hours. If you do not receive a confirmation, please contact the Executive Hearing Office at 602.712.7737 to ensure that your submission was received.
