$25 initial application fee/ $25 annual renewal. Applicant must submit a copy of a current membership card or original letter from the Legion of Valor.

Special Plate Application (form #96-0143) is required. Also available with a disability symbol.  Not available to be personalized.

$17 dollars of the initial and annual renewal fee goes to the organization.
