Five-Year License
Military personnel and dependents stationed in Arizona may visit any MVD office to apply for or to renew a five-year license.
Replacement License
Replace your license online at AZMVDNow.gov.
Renew By Mail
If stationed outside Arizona, please submit the following:
- Notarized Driver License Application
- Photocopy of the front and back of your current driver license
- Photocopy of the front and back of your military ID card
- A check or money order for $10.00.

Military Name Change

Motorcycle Endorsement
If stationed outside Arizona, please submit the following:
- Notarized Driver License Application
- Photocopy of the front and back of your current driver license
- Photocopy of the front and back of your military ID card
- Motorcycle Safety Course completion card
- A check or money order for $7.00.

Reinstatement
Qualified applicants may reinstate a suspended driver license online at AZMVDNow.gov. A revoked license cannot be reinstated online. More information.
Photo Update
All licensees are required to update their photo and pass a vision test every 12 years. However, if there is active military status recorded, this requirement will not apply.
