Organ and Tissue Donor Program

Opt in to become an organ and tissue donor and give the gift of life. Your decision to donate can save up to eight lives and heal more than 75 people through tissue donation.

Become a Donor Online

The easiest way to register as an organ and tissue donor is online through AZ MVD Now. When you apply for a driver license or identification card online, simply check the organ donor box to:

• Add your name to the DonateLifeAZ Registry
• Get the organ donor designation printed on your license or ID card

Become a Donor at an MVD Office

You can also register when you visit any MVD office. Just check the organ donor box on your Driver License/Identification Card Application.

Managing Your Donor Status

Removing the Designation from Your License

If you currently have the organ donor designation on your license or ID card and want it removed, you can:

• Apply online at AZ MVD Now
• Visit any MVD office to make the request

Note: Removing the designation from your license doesn't automatically remove your name from the DonateLifeAZ Registry.

Updating Your Donor Registration

To modify or remove your registration from the DonateLifeAZ Registry, you have several options:

• Visit DonateLifeAZ.org/MVDprofile to manage your registration online
• Email [email protected]
• Call 1-800-94-DONOR (1-800-943-6667) to speak with a representative

Standard transaction fees apply when updating your driver license or identification card.
