Intrastate

Intrastate: This means you're either under the age of 21 (cannot drive CDL outside of AZ) or you have an Amputation or Limb impairment and want an Intrastate Waiver to be restricted to only drive in the state of Arizona.

Intrastate applies to drivers under the age of 21 or those with a Limb Impairment/Amputation

Limb impairment/amputation will need to apply with the medical review unit, you must submit a copy of a current DOT Medical Examination Report, Medical Examiner Certificate, a Medical Waiver Evaluation Summary and a completed Intrastate Waiver Application.
