Intrastate Waivers
Intrastate Vision Waiver Requirement
You must have driven the type of vehicle to be operated as stated in the waiver for at least two of the previous five years. You are prohibited from transporting passengers or hazardous material.
To apply, you must submit a copy of a current DOT Medical Examination Report, Medical Examiner Certificate, a current Vision Examination Report completed by an ophthalmologist or optometrist, and a completed Intrastate Waiver Application.
If you are found to be eligible for the waiver, you will be required to take the Special Performance Evaluation (SPE) road/skills test at an CDL office with a certified examiner prior to issuance of the waiver.
Intrastate Limb Impairment/Amputation Waiver
To apply, you must submit a copy of a current DOT Medical Examination Report, Medical Examiner Certificate, a Medical Waiver Evaluation Summary and a completed Intrastate Waiver Application.
If you are found to be eligible for the waiver, you will be required to take the Special Performance Evaluation (SPE) road/skills test at an CDL office with a certified examiner prior, to issuance of the waiver.
