Medical Review Suspension and Revocation

If you received a suspension and/or revocation notice, you may have

  • failed to comply with medical requirements.
  • failed to meet medical and/or vision standards.
  • failed to comply with a request for a vision, written or road test.
  • been ruled incapacitated by a court.

If your driving privilege is suspended and/or revoked for failure to submit a vision examination, medical examination, substance abuse evaluation, certificate of completion (COC) or a driving evaluation, you may submit the information to your nearest MVD office. You have the right to request a hearing on these actions; however, such a request will not stop the immediate suspension. Your request must be received within 15 days from the date of the notice.

The request must include your full name, date of birth, license number, mailing address, home and business phones, and case number. Upon receipt of your written request, a hearing may be scheduled and you will be notified as to the date, time and location.
