Medical Review Suspension and Revocation
Medical Review Suspension and Revocation
If you received a suspension and/or revocation notice, you may have
- failed to comply with medical requirements.
- failed to meet medical and/or vision standards.
- failed to comply with a request for a vision, written or road test.
- been ruled incapacitated by a court.
If your driving privilege is suspended and/or revoked for failure to submit a vision examination, medical examination, substance abuse evaluation, certificate of completion (COC) or a driving evaluation, you may submit the information to your nearest MVD office or by email. For any other suspension and/or revocation from the Medical Review Program, please call 602.771.2460 for further information.
You have the right to request a hearing on these actions; however, such a request will not stop the immediate suspension. Your request must be received within 15 days from the date of the notice. The request must be made in writing and may be made by any one of these methods:
- Online: Use our secure email response system, which encrypts your personal information.
- Mail: Send your request to this address:

Arizona Department of Transportation
P.O. Box 2100

Phoenix, AZ 85001 - Fax: Send your request to 602.241.1624.
- Personal Delivery: Visit the Executive Hearing Office at 3838 N. Central Ave., Suite 300, in Phoenix, or any MVD office statewide.
The request must include your full name, date of birth, license number, mailing address, home and business phones, and case number. Upon receipt of your written request, a hearing may be scheduled and you will be notified as to the date, time and location.
It is against the law to operate any motor vehicle on the public roads of this state while your driving privilege is suspended. If such operation results in your arrest and conviction, the suspension will be extended for up to one additional year.
