Application Process - Taxi

STEP 1:

Create a trade dress that will be displayed outside and inside your vehicle at all times while on duty.

STEP 3:

Submit a new application via DocuSign.
Include the following items with your application

• Articles of Incorporation or Registered Trade Name Certificate
• Photo(s) of Trade Dress install on the vehicle

Please only submit one application, you will be contacted via email once the application is processed.

STEP 4:

ServiceArizona will activate your permit once your application is completed

STEP 5:

Log on to your ServiceArizona Secure Access Gateway account and print your permit
