Fees

Payment may be made by credit card, cashier's check or money order. Cash is not accepted by mail. No personal checks are accepted for reinstatement.

Application Fees

• Age 50 and older: $10
• Ages 45-49: $15
• Ages 40-44: $20
• Age 39 and younger: $25

Note: Additional fees will be required to complete the reinstatement process.
