We want to make sure every driver on Arizona roads is capable of safely operating a vehicle. That's what the Medical Review Program is all about.
We work with the Medical Advisory Board and the Arizona Office for Americans with Disabilities.
When you might be reviewed
You may be reviewed based on:
- A medical condition you report
- A report from law enforcement
- A report from a medical professional
- A report from an MVD office
- A report from a concerned community member

We review each report to see if you need a re-examination of your driving skills, a written test, and/or a medical or psychological evaluation.
Your responsibility
You are responsible for reporting any medical condition that may affect your ability to drive safely. Report this information as soon as your medical condition allows.
If you hold a Commercial Driver License (CDL), check the Commercial Services section for medical review information specific to your license.
If you receive a testing notice
If you receive an Administrative Action notice, complete the required tests; no appointment needed. Visit any MVD or Third Party office during normal business hours.
Re-examination notice (vision screening, vehicle control skills test, knowledge test, or road test): Complete all testing requirements within 45 days of your notice.
If you do not complete the required testing within the allowed time, your driving privilege will be suspended or revoked.
If your driving privilege is suspended or revoked
You may have received a suspension or revocation notice if you:
- Failed to meet medical standards
- Failed to comply with medical requirements
- Failed to comply with re‑examination testing requirements
- Were ruled incapacitated by a court
If you failed to submit required documentation
If your driving privilege was suspended or revoked for not submitting a medical examination, medical substance abuse evaluation, or driving evaluation, you can submit this information to [email protected], or at your nearest MVD or Third Party office. You may also submit an SPE certificate from one of the following institutions:

Your right to request a hearing
You have the right to request a hearing on these actions. However, requesting a hearing will not stop the immediate suspension. You must submit your request within 15 days from the date of your notice.

Include your full name, date of birth, driver license number, mailing address, home and business phone numbers, and withdrawal number. The hearing office will schedule a hearing and notify you of the date, time, and location.
Window tinting exemption
Arizona law determines how much and what kind of window tinting your vehicle can have. You can find these window tinting requirements in Arizona Revised Statute 28-959.01. Operating a vehicle that does not meet these requirements violates the law.
If you have a medical need for protection from direct sunlight, you can receive an exemption from the standard window tinting requirements. The exemption applies to any vehicles you own and other vehicles where you are a regular passenger.
To apply for an exemption, submit a completed Application for Window Tint Medical Exemption certified by a Doctor of Medicine (MD), Doctor of Osteopathic Medicine (DO), Naturopathic Medical Doctor (NMD), or Ophthalmologist to the Medical Review Program. You must have a certificate in each vehicle you own.
