If you would like to be added to the DonateLifeAZ Registry and have the ——printed on your driver license or identification card, you may apply online at AZMVDNow.gov or go into any MVD Office, please check the appropriate box on your Driver License/Identification Card Application.  

If you currently have thedesignation on your driver license or ID card and would like it removed, may apply online at AZMVDNow.gov or go into any MVD Office to make the request. A new license or identification card will be mailed to you without the designation, but you are not removing your name from the DonateLifeAZ Registry. To modify or remove your registration, visit DonateLifeAZ.org/MVDprofile, email [email protected], or call 1-800-94-DONOR.
