ADOT and Non-ADOT Users will use this form to request access to the Arizona Crash Information System (ACIS).

1. Reach out to ADOT’s Joint Project Agreement (JPA)group at [email protected] to have an IGA created or amended or,

2. Create an account with ADOT Public Records to submit a request to receive crash data records.
