$25 initial application fee/ $25 annual renewal. Applicant must be a veteran or immediate family member of a person issued a Veteran license plate. Immediate family member is a spouse, parent, child, brother or sister.

Special Plate Application (form #96-0143) is required. The standard size plate may be personalized with a maximum of 6 characters.

Veteran must provide a copy of a DD214, 215, 2 (retired), 2 (reserve) or 1173 or 256A; or provide an original statement of honorable service from the US Department of Veterans Affairs or the Arizona Department of Veterans' Services or any one of the following cards: American Legion, Arizona credential with Veteran distinguishing mark, Department of Veteran Affairs veteran identification, Disabled American Veterans, Military Officers Association of America, Military Order of the Purple Heart, Veterans of Foreign Wars, Vietnam Veterans of America, or U.S. military ID (active or inactive duty).

This plate may be ordered online, if the required documentation has been submitted and your driver record indicates veteran status, or if you have already been issued a Veteran special plate.

Also available in a small size plate for motorcycles and golf carts. May be personalized with a maximum of 4 characters.
