The disability plate or placard permits the recipient to park in specially marked spaces provided for the exclusive use of persons with a disability. The person with a disability must be the driver or a passenger in the vehicle when the disability parking space is used. Disability plates can only be displayed on a specific vehicle registered or leased by the person with the disability.

Recipients must complete a Disability-Hearing Impaired Plate/Placard Application or have one on file.

Temporary Disability Placards

Temporary disability placards are valid for six months at which time a new certification must be completed.

Hearing Impaired

The Hearing Impaired plate is in a special, reserved number format beginning with the letter "H." Hearing Impaired plates and placards alert law enforcement, emergency services and others approaching the vehicle that the driver is hearing impaired. Hearing Impaired plates/placards are not valid for special parking privileges. 

Medical Certification

The Medical Certification section on the application must be completed by an authorized physician (doctor of medicine, osteopathy, podiatry or chiropractic, licensed to practice medicine in the United States), an authorized physician assistant, a registered nurse practitioner or by a hospital administrator. Plates are assigned to, and may only be displayed on, a specific vehicle registered or leased in the disabled applicant’s name.

If leased, but not registered in the applicant’s name, a document showing the disabled applicant as a lessee is required. A disability plate may be transferred to another qualifying vehicle by visiting the nearest MVD or Authorized Third Party Office.

For parking in reserved parking spaces marked with the International Symbol of Access. No fee is required to add a disability symbol to selected plates where indicated (other special plate fees still apply.

Parking spaces marked with this symbol are only for use by a vehicle displaying a valid disability plate or placard, and only when transporting the person who was issued the plate or placard.

Must complete application form #96-0104 signed by a physician or hospital administrator.

Applicant must have one or more of the following conditions: unable to walk 200 feet without stopping to rest; unable to walk without assistance from another person or from a brace, cane, crutch, wheelchair or other prosthetic or assistive device; lung disease with forced respiratory, expiratory volume for one second, if measured by spirometry, is less than one liter, or the arterial oxygen tension is less than 60 mm/hg on room air at rest; uses portable oxygen; cardiac condition with Class 3 or 4 functional limitations as by American Heart Association standards; or severely limited in ability to walk due to an arthritic, neurological or orthopedic condition.

Alternative Fuel Specialty Plate

1 Plate Issued

$8.00 initial application fee/ no annual renewal fee. Must be powered exclusively by liquefied petroleum gas(LPG, propane), natural gas(CNG/LNG), a blend of 70% alternative fuel and 30% petroleum-based fuel, hydrogen, solar, or electric (excluding golf carts). May be personalized with a maximum of 7 characters.

Amateur Radio License Plate

1 Plate Issued

$25 initial application fee/ $25 annual renewal. Applicant must have a valid amateur operator license from the Federal Communications Commission. The plate number is the FCC assigned call sign.

$17 dollars of the fee goes to a special fund for college scholarships, youth programs, and emergency response communications.

Special Plate Application (form #96-0143) is required.

$17 goes to the Arizona FFA which provides premier leadership, personal growth and career success opportunities for students enrolled in Agricultural Education programs in Arizona. May be ordered online. May be personalized with maximum of 7 characters.
